Buteyko method
The Buteyko method is a breathing practice from the 1950s by Konstantin Buteyko. It trains quieter nasal breathing and reduced over-breathing (hyperventilation). The aim is higher retained carbon dioxide (CO₂) in the blood and lungs.
Hyperventilation dumps CO₂. Low CO₂ tightens airways, impairs oxygen unloading into tissues, and raises inflammatory signals. See CO2. Two minutes of hard over-breathing already raises lung leak and lifts serotonin and histamine.[1]
Simply hyperventilating for a couple of minutes will increase leakage from the blood into the lungs; hyperventilation decreases carbon dioxide, and increases serotonin and histamine.
Practice
[edit]Nasal breathing warms, humidifies, and filters air. Mouth breathing is cut. Breathe through the nose, keep the volume low enough to feel a mild hunger for air, slow the rhythm, let the diaphragm move, and stay relaxed.
Reduced breathing: smaller volumes, gentle breath holds after exhale (control pause). The control pause is how long a person can hold after a normal exhale without strain. Longer pauses mark higher CO₂ tolerance in the method's own language.

Other reduced/paced-breathing techniques, not part of Buteyko itself but built on the same over-breathing/CO2 logic:
- 4-7-8 breathing: inhale through the nose for 4 seconds, hold for 7, exhale slowly through pursed lips for 8. Developed by Andrew Weil, adapted from pranayama. Used for quick calming and for falling asleep.[3]
- Nose-unblocking exercise: breathe out normally, then hold the breath with the nose pinched shut while gently nodding or shaking the head until a moderate air hunger builds, then breathe back in through the nose only. Relaxation interrupts panic-driven over-breathing during symptoms.
The method grew inside Soviet clinics for asthma and related conditions. It later spread to Australia, New Zealand, the UK, and the US.[5]
Evidence (clinical)
[edit]A 1980 Moscow pediatric trial and a 1994 Brisbane randomized trial reported large cuts in bronchodilator and steroid use and in asthma symptoms among Buteyko trainees versus controls (Brisbane: bronchodilator use down 96%, steroids 49%, symptoms 80% in the method arm as reported in secondary summaries).[6]
A 2020 Cochrane review found moderate- to low-certainty evidence for better asthma quality of life and symptoms. Trial quality is limited. Mainstream medicine still ranks it as complementary care.[7]
Ray Peat noted that Jane Brody, the New York Times health writer, favorably covered Buteyko for asthma, alongside the related concept of permissive hypercapnia (deliberately tolerating higher CO2 during mechanical ventilation to avoid lung damage from excess oxygen).[8]
Link to bioenergetics
[edit]Retained CO₂ supports the Bohr effect (oxygen delivery into tissues), vessel tone, and barrier stability. Sugar oxidation produces more CO₂ than fat oxidation.[9] Buteyko is the main Soviet clinical practice that aims at that CO₂ set-point. Chronically overactive carotid body chemoreceptors (which sense low oxygen, high CO2, and low pH) are implicated in insulin resistance and metabolic disease; animal work shows that reducing carotid body activity improves glucose handling and reduces sympathetic overactivation, and human carotid body overactivity is reported in prediabetes.[10] Since Buteyko practice trains higher CO2 tolerance, proponents argue it may act on this same chemoreceptor pathway, though this specific link has not been tested directly in Buteyko trials.
