Coffee

Coffee is a practical nutrient source rather than only a caffeine delivery vehicle.[1]

Cognitive and mood

Chronic unpredictable stress (mice)
[edit]In adult mice, three weeks of chronic unpredictable stress (CUS) reduced weight gain and produced anxiety-like behavior, anhedonia (lower sucrose preference), helplessness (forced swim), reduced self-care (splash test), and spatial-memory deficits. Free access to caffeinated coffee (1 g/L, night period), but not the same concentration of decaffeinated coffee, attenuated those CUS-induced mood and memory changes and limited the stress-related drops in hippocampal long-term potentiation and frontocortical BDNF. The authors point to caffeine as the coffee constituent most responsible for the effect.[9]
Dementia risk
[edit]In a pooled analysis of the Nurses' Health Study and Health Professionals Follow-up Study (131,821 participants followed up to 43 years), the highest quartile of caffeinated coffee intake carried 18% lower dementia risk than the lowest quartile after adjusting for confounders (hazard ratio 0.82, 95% CI 0.76-0.89). Decaffeinated coffee showed no such association, and tea intake tracked similarly to caffeinated coffee.[10] See Dementia.
Coffee rapidly lowers prolactin.

Cancer prevention
[edit]Coffee reduces fat via lowered lipogenesis its components (CGA, caffeine, etc.) inhibit enzymes like acetyl-CoA carboxylase and fatty acid synthase.[13] Fatty acid synthase (FASN) aids tumor growth/survival in most cancers, interacts with cancer networks, and is a universal drug target for treatment/prevention.[14]
Colon cancer recurrence
[edit]In a secondary analysis of 953 stage III colon cancer patients from the CALGB 89803 (Alliance) trial, drinking 4 or more cups of total coffee daily during and after adjuvant chemotherapy lowered the hazard of cancer recurrence or death by 42% compared with never drinkers (HR, 0.58; 95% CI, 0.34 to 0.99; Ptrend = .002). At the same intake, caffeinated coffee specifically showed a larger reduction (HR, 0.48; 95% CI, 0.25 to 0.91; Ptrend = .002), and caffeine intake alone (highest versus lowest quintile) also reduced recurrence or death (HR, 0.66; 95% CI, 0.47 to 0.93; Ptrend = .006). Decaffeinated coffee and nonherbal tea showed no association with outcome.[15]
Fibrocystic breast disease
[edit]The idea that caffeine causes fibrocystic breast disease traces to a single, methodologically flawed source: a widely publicized series of articles by J. P. Minton, whose case rested on breast tissue with more disease having higher cyclic AMP, combined with caffeine raising cyclic AMP. He missed that healthy breast tissue is mostly fat, and fat cells run low in cyclic AMP regardless of disease state, so his measurement tracked how much glandular tissue was in the sample, not how diseased it was. Other substances that inhibit cancer growth also raise cyclic AMP, while estrogen lowers it and increases cell growth; argued consistently from his own logic, Minton's conclusion should have called for more caffeine, not less.[16]
Telomere length

Sleep
[edit]"Sleep quality and duration predict presence of pain the next day in healthy subjects, suggesting that sleep disturbances alone may worsen pain, and experimental sleep deprivation in humans... Caffeine and modafinil, two wake-promoting agents that have no analgesic activity in rested mice, immediately normalize pain sensitivity in sleep-deprived animals, without affecting sleep debt."[18]
"Individuals who habitually consume greater amounts of caffeine as part of their daily diets demonstrate diminished sensitivity to painful stimuli in a laboratory setting."[19]
Obviously, you don't want to dose caffeine in such a way that it interferes with sleep...
Pain sensitivity
[edit]Not getting enough sleep increases pain sensitivity[20] however caffeine significantly reduces heat and pain sensitivity.[21]
Good source of magnesium
[edit]Ray said he brewed VERY strong coffee using a specific pour-over method to extract the most out of the fine grounds. This would result in an extremely concentrated thick brew, probably more similar to an espresso than a standard cup of coffee. This makes it a very rich source of magnesium.[22]
Dark roast provides the most niacin of the common roast levels.[23]
Dosing
[edit]Three to five cups a day is a reasonable general target; studies following people who drink more than five cups a day found lower incidences of various cancers and lower dementia rates, with the benefit still increasing across that range rather than flattening out.[24]
It doesn't hurt to drink 50 if that balances your metabolism, but I think everyone should try to get three to five cups a day.

Need varies with metabolic state. When severely hypothyroid, higher intake (30-50 cups) matched how much a very low metabolic rate was pulling toward adrenaline compensation; correcting thyroid directly is still the more sustainable fix (see Thyroid), but the dose range above reflects that coffee's right amount tracks metabolic need rather than a fixed ceiling.[24]
Timing
[edit]A 2025 analysis of 40,725 US adults (NHANES 1999-2018, validated against a separate 1,463-person cohort) identified two coffee-drinking patterns: morning-type (most intake before noon, 36% of drinkers) and all-day-type (14%). Over a median 9.8-year follow-up, the morning-type pattern was associated with 16% lower all-cause mortality (HR 0.84) and 31% lower cardiovascular mortality (HR 0.69) than not drinking coffee at all. The all-day-type pattern showed no such association. Higher coffee intake amounts tracked with lower all-cause mortality only in the morning-type group, not the all-day-type group.[25]
Tolerance and adjustment
[edit]Anxiety, shakiness, or a racing pulse after coffee usually reflects the starting metabolic state, not a fixed reaction to caffeine itself.
Low glycogen and an empty stomach
[edit]Caffeine raises metabolic rate and glucose consumption taken on an empty stomach, the resulting drop in blood sugar triggers a compensatory adrenaline release, producing the anxious, shaky reaction commonly blamed on caffeine directly.[26]
It's important not to drink coffee on an empty stomach, it should always be with food, since it increases the metabolic rate, and can deplete glycogen stores.
Often, a woman who thinks that she has symptoms of hypoglycemia says that drinking even the smallest amount of coffee makes her anxious and shaky. Sometimes, I have suggested that they try drinking about two ounces of coffee with cream or milk along with a meal. It's common for them to find that this reduces their symptoms of hypoglycemia, and allows them to be symptom-free between meals.
Starting with a small amount, around two ounces, always with a meal and some milk or sugar rather than black on an empty stomach, keeps blood sugar from dropping and avoids the adrenaline spike.
Thyroid and adrenal status
[edit]Jitteriness from a small amount of coffee is commonly a sign of an already-stressed system compensating with adrenaline for low thyroid output, rather than a problem with coffee. See Thyroid, Adrenaline, and the pulse/temperature markers on Roadmap/02 - Self diagnosis & markers.
I suspect that it happens mostly with hypothyroidism, because in the 1970s I averaged dozens of cups a day, and thought about it as soon as I woke up, then suddenly after I took some thyroid, I didn't feel any need for it.
Correcting the underlying thyroid deficit changes how coffee is tolerated the goal is fixing the metabolic rate, not just avoiding the trigger.
Coffee and TSH
[edit]In a 2023 analysis of NHANES 2007-2012 adults (≥20 years, n=6,578) plus Mendelian randomization, people drinking 2-4 cups of coffee per day had lower serum TSH than non-drinkers (b=-0.23, 95% CI: -0.30 to -0.16), with no significant change in TT4, FT4, TT3, or FT3. Drinking under 2 cups daily was associated with lower odds of subclinical hypothyroidism (OR=0.60, 95% CI: 0.41-0.88). Neither the observational models nor MR linked coffee intake to overt hyperthyroidism or hypothyroidism.[30] Peat's caffeine essay notes that coffee drinkers have a lower incidence of thyroid disease, including cancer, than non-drinkers.[31]
"Adrenal fatigue"
[edit]"Adrenal fatigue" as a popular term has no basis in Hans Selye's original stress research, the source it's usually credited to. Selye's General Adaptation Syndrome described a shock phase, an adaptation phase, and only under extreme, prolonged stress an "exhaustion" phase in which the animal died, not a partial, chronically tired gland.[32][33]
Moderately stressed adrenal tissue regenerates rather than wearing out: removing the contents of the adrenal cortex leaves the fibrous capsule's inner lining able to multiply and rebuild a fully structured new gland within about three months.[32]
What actually fails under chronic stress is thyroid output, not the adrenal glands themselves. Stress lowers thyroid function, and the adrenals need thyroid hormone to convert cholesterol into cortisone; low thyroid breaks that conversion, producing what looks like "adrenal failure" but is functionally thyroid failure.[32]
Selye also found that pregnant rats didn't need their adrenal glands: the next stress after giving birth would normally kill an adrenalectomized rat, but a daily dose of progesterone after removing the adrenals let them live a full, healthy life, since progesterone compensated for the missing adrenal output.[32]
Gut inflammation and skin reactivity
[edit]Coffee's acidity and stimulant load land differently on an already-inflamed gut. Intestinal irritation is a common driver of skin reactions generally, including flare-ups that look like eczema.[34] Addressing the underlying gut irritation, see Gut for the endotoxin mechanism and practical approaches, improve coffee tolerance, rather than cutting coffee alone fixing the skin.
Easing in
[edit]- Always with a meal, never black on an empty stomach
- Add milk, cream, or sugar to blunt the metabolic-rate spike and buffer acidity
- Start small (two ounces) and increase gradually rather than jumping to a full cup
- Lower-temperature brewing extracts less caffeine per amount of minerals and vitamins than hot, high-temperature methods[35]
- Gelatin/broth alongside coffee supplies glycine, which has calming, anti-inflammatory, GABA-supportive actions that can offset caffeine's stimulating effect while gut and adrenal function are still recovering
- Coffee limits iron absorption when drunk with an iron-containing meal (oysters, eggs, meat), useful for anyone managing high ferritin or iron overload.[36]
- Running mostly on coffee and sugar in place of real food raises nutrient needs (both increase metabolic rate) while narrowing intake; biotin, vitamin B6, pantothenic acid, selenium, and copper are typically the first to run short under that pattern.[37]

Preparation
[edit]Drip method binds metals from city water in the grounds. Strong drip coffee is the usual brew. Lower-temperature methods extract less caffeine per minerals and aroma than very hot espresso-style extraction.[38] Prefer paper or metal filters that do not add unwanted residues; avoid filters with plastic taste or unknown coatings when possible.
Decaf
[edit]Decaf is a legitimate nutrient source if it's decaffeinated with CO₂ rather than a chemical solvent; either way it still supplies potassium, magnesium, niacin, and other B vitamins.[39]
